Requirements
- Accounting records as of 6 months prior to the incident, certifying direct and indirect costs, per unit of production and by references.
- Current income statement that evidences production inventory.
- Raw material purchase invoices for the manufacture of products, as of 6 months prior to the date of the loss.
- Monthly sales supporting documents as of the last 6 months prior to the date of the event, they must include the sale price per unit of production and references, signed by an accountant and attaching a copy of professional card.
- Copy of the machines daily production controls, as of 6 months before the accident.
- Production orders ceased during the days claimed or unfulfilled programming during those days.
- Description pertaining the personnel assigned to the affected machines on said days, which ceased working on claimed days and payroll costs including basic salary.
- Copy of the maintenance log for said machines.
- Profit margins per production unit.
- Latest statements of ICA, VAT and excise and liquor tax.
- Indicate schedule of the affected activity.
